Lockport Township is an outreach center for Senior Services Center of Will County. Senior Services Center of Will County provides information, referrals to community resources, and access to long term care services. Information provided could include housing options, in home care services, assistance with completing applications for BEAM Medcaid, low income subsidy, Medicare Part D, energy assistance, and information on any concern/need of an elderly person. Their goal for infomation and assistance is to empower individuals to make informed choices and to streamline access to services.A Community Resource Worker is available to provide assistance with any questions, conerns, or completing applications at their main office in Joliet or at the Lockport Township outreach center. Community Care Program Guidelines And Qualifications

– 60 years of age or older

– Illinois Resident

– U.S. Citizen or Legal Alien

– Score of 29 Points on the DON – the DON is part of the in home assessment and will be explained in detail by the case manager at the home visit

Have non-exempt assets of $17,500 or less

  • Your home, car, or personal furnishings are classified as exempt assets.
  • The assets that are counted are Checking and Savings account balances, cash on hand, life insurance policy with a cash value, prepaid burial policy, CDs, stocks, bonds, trusts, IRAs, annuity, real estate other than the primary residence, and vehicles other than the primary vehicle.

What are the asset levels?

  • Single person – $17, 500
  • Couple where only one person is going to be receiving services – $109,560

  • A Medicaid application will need to be completed if an exemption is not met exemptions are based on asset level.

  • Senior Citizen Assessment Freeze Homestead Exemption

    The Assessment Freeze Homestead Exemption provides seniors with limited income protection against real estate tax increases due to rising property values. It is not a tax freeze or a tax reduction and does not protect against increased taxes due to tax rate increases. Because this exemption provides for a base year frozen assessment, it will potentially provide increased savings each year a senior is eligible. The base assessment used in the Senior Citizens Assessment Freeze Homestead Exemption initially equals the assessed value from the prior year’s tax calculation. In subsequent years, if the new assessed value is lower than the original base value, the new lower value becomes the new base amount.

    Apply For The Senior Citizen Water Bill Discount

    If you are a senior citizen living in Philadelphia, you may qualify for a 25% discount on your water and sewer bill. To receive the discount, you must:

    • Be at least 65 years old.
    • Live at the address listed on the application.
    • Have the water and sewer bill in your name.
    • Have a total annual income of $32,300 or less.

    To apply for the discount, you must complete an application form. Youll also need to provide photocopies of documents that establish proof of your age, address, and household income. Applications require two separate proofs of residency. Examples of acceptable documents are listed on the application form.

    Adult Day Service

    • Designed especially for older adults who want to remain in the community but who cannot be home alone during the day due to a physical, social and/or mental impairment
    • Provides respite for family caregivers, especially those who are employed outside the home, and socialization for isolated older adults
    • Services offered in adult day service centers include health, medication supervision, personal care and recreational/therapeutic activities
    • Nutritious lunches and snacks are served, and special diets are provided

    Automated Medication Dispenser Service

    Emergency Home Response System

    • EHRS is a 24-hour emergency communication link to assistance outside the home for older adults with documented health and safety needs and mobility limitations
    • This service is provided by a two-way voice communication system consisting of a base unit and an activation device worn by the client that will automatically link the older adult to a professionally staffed support center

    In Home Service

    • In-Home Service provides assistance with household tasks such as cleaning, planning and preparing meals, doing laundry, shopping and running errands. Homemakers also assist clients with personal care tasks, such as dressing, bathing, grooming and following special diets

    Senior Companion

    • Scheduled visits by an older adult to provide friendship, companionship, and social activities

    What Is The Community Care Program

    The Illinois Department on Aging’s Community Care Program helps seniors to stay in their own homes by providing in-home and community based services. The Community Care Program aims to help seniors maintain their independence as an alternative to nursing home placement. The Community Care Program will provide services to a senior that meet all the current eligibility requirements.

    What the Community Care Program is not…

    • The Community Care Program is not 24 hour care.
    • The Community Care Program also cannot assist with medical needs or care.

    Senior Services Of Will County Offering Assistance For Elderly With Health Department Covid Vaccine Registration Survey

    The Will County Health Department and Senior Services of Will County have formed a partnership intended to assist senior residents of Will County with the COVID Vaccination Survey on WCHDs website.

    Due to the fact that some senior residents have had some difficulty registering, WCHD has accepted assistance offered by Senior Services of Will County. Will County senior residents may call Senior Services at 815-723-9713 for staff assistance over the phone. If a Senior Services volunteer is not available at that time, the callers phone number will be taken, and they will be called back soon by a volunteer.

    THIS NUMBER IS NOT FOR VACCINE INFORMATION OR TO MAKE VACCINE APPOINTMENTS. IT IS SIMPLY TO HELP ANY RESIDENT REGISTER THEIR INTEREST IN GETTING THE VACCINE WHEN AN APPOINTMENT IS AVAILABLE IN THE FUTURE FROM THE WILL COUNTY HEALTH DEPARTMENT.

    This survey, already used by more than 70,000 Will County residents, is designed to help those who plan on taking the COVID vaccine in registering their interest with WCHD, as well as providing important information such as occupation, health factors, and if they are 65 or over. Residents will be contacted when a COVID vaccination clinic is available for them, and they can make an appointment at that time.

    Although Will County is still currently in Phase 1A, this partnership will allow the county to be more prepared for Phase 1B, which will include the vaccinations of those 65 and over.
